Refactor remaining Project and Build files

- Simplify project files
  - Add and Update comments
  - Remove unnecessary lines
  - Rearrange and Order code blocks

- Update Git Ignore to latest
- Rename NuGet Icon to `Icon.png`
- Normalize casing for `ReadMe.md`
- Rename solution file to `CommunityToolkit.sln`
